Who Is Eligible for Hearth’s Services?

You are eligible for Hearth’s services if you have a life-limiting illness and have chosen care that focuses on symptom management rather than curative treatment.

Diagnosis commonly associated with our services include:

  • Alzheimer’s disease
  • Cardiovascular disease
  • Liver/Renal disease
  • Cancer
  • Neurological disease
  • Respiratory disease
  • Stroke or Coma
  • Other life-limiting illnesses

Individuals eligible for Hearth’s services may experience some or all of the following:

  • Frequent hospitalizations
  • Deteriorating mental abilities
  • Recurrent infections
  • Decreased ability to perform Activities of Daily Living (ADLs)
  • Other specific decline in condition

For those that have never experienced hospice personally, they may not have a full appreciation for the extensive range of services we offer and our support for patients AND caregivers. However, for many, even the word “hospice” can be a challenging topic.

The Bridge of Hope Coordinator serves as your guide, helping you identify needs, providing emotional support for decision making and pointing clients toward valuable community services.

Getting Started

A consultation with a Hearth team member is available anytime, free of charge. This consultation will provide general hospice information, help to identify plan of care goals, assess patient and family needs and give comfort in knowing your loved one will be taken care of.
